Thank you for taking the time to tell us about your experience. So that we can pass your compliment on to the relevant staff or clinical area, it will be helpful if you can provide us with as much detail as possible. You can do this via a number of routes, including:

• In writing
• By telephone
• By email
• Face to face

Wherever possible, please let us know when and where events took place – e.g. which hospital or clinic, and the date and time or day of the week. These details are helpful because we have many hospitals and several wards in different hospitals have the same number.
Where possible, it would be helpful if you can provide us with the following information:

• Are you the patient (yes or no).  If no, please give your name and relationship to the patient
• Patient’s name
• Patient’s postal address including post code
• Your email address, if you have one
• Patient’s date of birth
• Brief description of compliment
• Location
• Date

Please include your contact details (a postal address, email or telephone number) so that we can get in touch with you if we need further information.
