As part of the discharge planning process, the Community Rehabilitation Service (CRS) provides short term rehabilitation (for up to 5 weeks) for people who have been in hospital.
The service is jointly funded and has staff from both NHS Lothian and The City of Edinburgh Health and Social Care Department.
An assessment is carried out by Occupational Therapists and Physiotherapists and a treatment programme is then devised in conjunction with clients to achieve their own goals.
The support staff can visit the clients 7 days per week.
In order to facilitate discharge home from hospital, referrals are only accepted from hospital staff.
Clients receive treatment and support in their own homes
